Lebanon restaurant inspections: Mold among 13 violations at city pizza shop - Lebanon Daily News

A city pizzeria is out of compliance, but passed previous inspections with flying colors.

Bella Lebanon Pizza, 701 Lehman St., Lebanon, was out of compliance with food safety laws during a regularly scheduled inspection on Oct. 4, according to a City of Lebanon report.

The pizzeria had 13 violations:

  • Raw chicken was stored above ready-to-eat items in the walk-in cooler.
  • A cardboard box full of garlic sauce containers was very moldy and sitting on the top shelf of the walk-in cooler (discarded). A piece of cardboard used for lining a shelf in the walk-in cooler is very moldy. Walls of the walk-in cooler, particularly behind the evaporator, are moldy.
  • Rear door located in the storage area of the food facility has a large gap all across the bottom and does not protect against the entry of insects, rodents, and other animals.
  • Personal items are stored on a shelf above the food prep area.
  • Food facility has a reminder statement on the menu for foods that are or could be served raw or undercooked to the consumer; however, does not clearly disclose which foods the reminder statement applies to.
  • Holes in the floor around water lines are not sealed, a downspout by the rear door does not direct water away from the building, breakers are not identified in two panel boxes and one panel box has an open slot. Two toilets stored in the rear area outside the food facility need to be removed.
  • Scoop being stored in the ice machine with handle buried beneath the ice. Food dispensing utensil in a container of peppers observed stored in the food and the handle was in the food.
  • A dirty bucket of stagnant water that had been catching a leak in the walk-in cooler was observed on the top shelf of the cooler. Removed. Observed cartons of soda mix stored directly on the floor in basement area, rather than 6 inches off of the floor as required.
  • Shelves throughout the facility are lined with cardboard. These shelves have non-food contact surfaces exposed to splash, spillage, or other food soiling; cardboard is not a cleanable impervious surface. Shelves in a reach-in refrigerator and the walk-in cooler are rusted and deteriorating.
  • Potential rodent harborage areas on the exterior of the building perimeter observed in rear area due to trash, leaves, skids and other debris.
  • An open employee's open soft drink beverage container was observed shoved down into the ice bin.
  • Cooked ground beef, potentially hazardous ready-to-eat food prepared in the food facility and held for more than 48 hours, located in the walk-in cooler, is not being date marked.
  • Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration.

Bella Lebanon Pizza was also out of compliance on July 19 due to the lack of a soap dispenser and towel dispenser in the food prep area. The issue was corrected July 31.

Otherwise, the pizzeria had a grand total of six violations in six inspections between 2017 and July 2019, and zero violations in its regularly scheduled inspections in 2017 and 2018.

Lebanon County Career & Technology Center, 833 Metro Drive, Lebanon

Regular inspection, two violations:

  • Several metal and plastic food containers on storage shelf were observed to have food residue and were not clean to sight and touch.
  • Non-food contact surfaces, such as shelves in store room, not cleaned at a frequency to preclude accumulation of dust and debris.

Inspected Sept. 23:

J M Mart, 102 E. Main St., Palmyra

Regular inspection, two violations:

  • Observed one half-gallon of one-percent milk being offered for sale with a sell by date Sept. 21, 2019. Corrected on site.
  • Lights are not shielded or shatter proof in the walk-in cooler.

Lisa's Café, 600 E. Main St., Palmyra

  • Mechanical warewashing equipment observed not being operated according to the manufacturer's data plate. The final rinse temperature is not reaching 180 degrees.
  • Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. The walk-in cooler observed to have dust accumulation on the fan guards and ceiling.

Inspected Sept. 24:

Bell & Evans Retail Outlet, 154 W. Main St., Fredericksburg

Regular inspection, no violations.

Blue Mountain Golf Course, 628 Blue Mountain Road, Fredericksburg

Regular inspection, no violations.

Smokin' Stuff BBQ (mobile food facility)

Opening inspection, no violations.

Inspected Sept. 25:

A&M Pizza, 626 Quentin Road, Lebanon

Regular inspection, four violations:

  • Paper towel dispenser missing at the handwash sink in the food service area.
  • Tools, cleaning items and personal items were stored on a small shelf above a food prep area. Corrected.
  • Menu is missing the reminder statement for foods that could be served undercooked to the consumer.
  • One spray bottle in prep area, used for storing cleaner taken from bulk supplies, was not marked with the common name of the chemical.

Francisco's Pizza, 139 E. Main St., Fredericksburg

Regular inspection, three violations:

  • Commercially processed refrigerated, ready-to-eat, time/temperature control for safety food (non-exempt cheese and deli ham), located in the walk-in cooler, and held more than 24 hours, is not being marked with the date it was opened. Corrected on site.
  • In the basement, delivery door and window has a gap, allowing light from the outside to show through to the inside, which does not protect against the entry of pests.
  • An open employee's beverage container was observed on the food preparation station in the back room. Corrected on site.

Fredericksburg Elementary School, 119 E. Walnut St., Fredericksburg

Regular inspection, no violations.

Frog's Hollow Tavern, 2271 Route 72, Jonestown

Regular inspection, one violation:

  • Food facility is not complying with "Boil Water" advisory issued by the Pennsylvania Department of Environmental Protection and is using water which does not meet standards for drinking water. Corrected.

Giant Food Stores, 481 W. Penn Ave., Cleona

Regular inspection, four violations:

  • Observed clean food equipment and/or utensils in produce area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting).
  • Food employees, observed in seafood area, not wearing proper beard cover.
  • Ice machine not cleaned at a frequency to prevent the presence of mold.
  • Wall in meat room in disrepair.

Northern Lebanon High School, 345 School Drive, Fredericksburg

Regular inspection, one violation:

  • Observed loose, peeling paint from the wall at the entrance side of the mechanical dishwasher.

  • An irreversible registering temperature indicator is not provided for measuring the utensil surface temperature for the mechanical dishwasher.
  • Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration
  • The food facility does not have the certificate for the certified food protection manager posted in public view.

Justino's Pizza, 28 E. Main St., Newmanstown

Regular inspection, no violations.

Lincoln Republican Club, 21 S. Ninth St., Lebanon

Regular inspection, five violations:

  • Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Employees are not familiar with the use of sanitizer test strips or test kit to determine sanitizer concentration.
  • The interior of the ice machine, a food contact surface, was observed to have a thick black residue and was not clean to sight and touch.
  • The food facility does not have a handwash sink located in the food and beverage service area/glass washing area.
  • Women's toilet room is not provided with a covered waste receptacle for sanitary napkins.
  • No sign or poster posted at the handwash sink in the men's toilet room to remind food employees to wash their hands.

Speedway, 600 E. Linden St., Richland

Regular inspection, two violations:

  • Floor under shelving in walk-in cooler has build-up of food residue and debris.
  • Plumbing system not maintained in good repair – observed drain leaking at the three-bay sink, and shut-off valve near the mop sink.

The Pennsylvania Department of Agriculture states all inspections are a "snapshot" of the given day and may not be representative of the overall, long-term cleanliness of an establishment.
